How to obtain a copy of a police, traffic, or incident report, background check, or letter of good conduct?
Go online to https://www.phila.gov/services/crime-law-justice/get-a-copy-of-a-public-safety-report/ for copies of public safety records. 
 
Appointments are again required for the recording of documents in person.
Email appointments.records@phila.gov to schedule.
All other Department of Records offices do not require an appointment.

Further Information:
To obtain a copy of a Police Report,
  1. Come to Room 170 in City Hall, 8:30 am - 3:30 pm, Monday to Friday (try to be there by 3 pm), to request an Incident Report. You can also ask for an application for a report to be emailed to you at policereports.info@phila.gov (215) 686-2266 or download it from here and then print it out, and/or
  2. Visit https://www.phila.gov/records/PoliceFire/Reports.html to download all applications/forms. Download form number 82-47.
  3. Mail your request to Police Record Unit, City of Philadelphia, Department of Records, Room 170, City Hall, Philadelphia, PA 19107. 
  4. $25 fee (non-refundable)
  5. Allow 10-12 weeks for the report, whether the application is made in person or by mail.
To obtain a copy of a Traffic Accident Report,
  1. Go Online to the Traffic Accident Report Portal.  Anyone who is eligible to obtain a traffic accident report can now do so online at https://crashreports.phila.gov. They can also be obtained by mail or by appointment like a Police Report.
  2. $25 fee (non-refundable)
  3. A government-issued ID is required for EVERYONE requesting traffic accident reports in person
  4. Allow at least 15 days for the report, whether requested in person, online, or by email.
The following persons are eligible for copies of reports:
  • Individuals involved in an accident
  • Automobile insurance companies with letter of release
  • Lawyers representing individuals involved in an accident with a government-issued ID
  • Other Governmental Jurisdictions
    Fire/ EMS reports:
    1. Fire Reports $20 (non-refundable) Click Here for the Fire Department website or go directly to public-safety-reports.phila.gov.
    2. EMS Reports $6.50 (non-refundable) Application for EMS Report
    3. Allow 6-8 weeks for the report.
    Police Records (background) Checks and Letters of Good Conduct are also available for activities within the City of Philadelphia.

    • Letters of Good Conduct:
      1. Download form number 75-343.
      2. $40 fee (non-refundable)
      3. Allow 10 days for the report
    Environmental Research Report 
    1. Visit https://www.phila.gov/records/PoliceFire/Reports.html to download all applications/forms and for further info.
