You may not have properly informed the Department of Revenue that your business is closed.
Please download and print the Tax Account Change Form from the Department of Revenue’s website: www.phila.gov/tax-change, also attached to this article.

Mail the completed change form to:
City of Philadelphia, Department of Revenue
P.O. Box 1410, Philadelphia, PA 19105-1410
Or call (215) 686-6600 to request a change form.
